What I want to make clear to you here is that The Issues List is about migrating a discipline and a culture down into your organization where EVERYBODY feels absolutely comfortable raising their hand and acknowledging, calling it out, saying “we’ve got an issue,” so that ALL your issues get on a list somewhere throughout the organization.
